Service 06
Support Coordination
Helping you make your NDIS plan work in everyday life.

For example: finding a new provider when your needs change, or getting ready for your plan review.
Support Coordination helps you understand your plan, connect with suitable supports, strengthen your service arrangements and build confidence to direct your own supports. The level of help depends on the Support Coordination funding in your plan and your situation.
Three levels of support
Helps you understand your plan, connect with community, mainstream and funded supports, and build confidence to manage your plan.
Helps you set up and keep effective supports, solve problems, strengthen relationships with providers and build the skills to direct your own support arrangements.
A higher level of coordination for people facing significant complexity, barriers or challenges in their support environment, with a focus on stability and consistent services.

We support NDIA-managed, plan-managed and self-managed participants. Our supports are charged in line with the NDIS Pricing Arrangements and Price Limits, and everything is agreed in a written Service Agreement before support begins.
